Thoracotomy & Extraction of humeral head.  (BOIS)

Hemiarthroplasty – PATEL

Proximal humeral hemiarthroplasty was chosen because the humeral head obviously had no intact blood supply.  Furthermore, there is significant technical difficult in fixing a comminuted 4-part proximal humerus fracture.  Attempts at open reduction and internal fixation in this setting are prone to failure due to tenuous fixation and avascular necrosis.
