Management
Initial management in the emergency room consisted of evaluation by ATLS protocols, administering supplemental O2, establishing adequate vascular access, and fluid resuscitation. Given the decline in cardiorespiratory status and the presence of hemopneumothorax a decision was made to perform a thoracotomy and lobectomy to remove the humeral head. The fracture was managed by proximal humeral hemiarthroplasty using the Global FX modular system (Smith and Nephew) which was performed via a ?deltopectoral approach?. A biceps tenodesis was also arried out and the tuberosities were bone grafted as per our usual protocol.
